- Attested copy of High School Certificate
- Attested copy of High School Marksheet
- Attested copy of Intermediate Marksheet
- Character Certificate from the last attended institution
- Caste Certificate, if required
- Six passport size photographs
- Self-addressed envelopes with required stamp
- Medical fitness certificate, if required
- Nursing registration certificate where applicable
